Case. A 48-year-old man presented with headache, left eye ptosis, a dilated pupil, and tooth pain for 10 days duration. He had a history of hypertension and poorly controlled type 2 diabetes.

Physical exam/diagnostic testing. Cranial nerve exam showed left eye ptosis and anisocoria. Consultation with ophthalmology specialists confirmed a left third nerve palsy and a possible concomitant fourth nerve palsy.

Hint. Although initial imaging was negative, further review showed erosion of the left carotid canal, consistent with an invasive tumor. An otolaryngology consultation led to a fine needle aspiration biopsy of an enlarged left cervical lymph node.
