Case. A 62-year-old man with a history of rheumatoid arthritis and gastroesophageal reflux presented to the hospital with persistent left leg pain and swelling for 1 day. The patient had tenderness of the left leg from ankle to thigh.

Diagnostic testing. CBC, comprehensive metabolic panel, and coagulation profile were within normal limits. CT venogram showed nonthrombotic occlusion and marked atresia of the left common iliac vein.

Hint. A few days prior to presentation, the patient reported having similar symptoms and was found to have a floating DVT in the left common femoral and greater saphenous veins for which he was started on outpatient anticoagulation.
